As a courtesy to you, we are happy to help you initiate your orthodontic insurance claim. We will provide you with an insurance form outlining treatment cost, down payment and length of treatment time.

We have found that if you deal with your insurance company directly, payment reimbursement for your claim is faster. In addition, your insurance policy is a contract between you and your insurance company. Therefore, if a problem should arise, it can be handled more efficiently by the policy holder.

From time to time, it will be necessary for us to verify continuation of treatment. We will be happy to complete the necessary forms and have them ready for you to return to your insurance company.

Your insurance claim will be paid directly to you rather than our office.
